We’re excited to offer you a reward for completing your Health Risk Assessment. Your reward may be a gift card or a 50 to 100 percent discount on your cost-sharing contribution, depending on your income. To qualify for a reward, your MI Health account needs to be current. It’s important to ensure timely payment of your copays and contributions.

This reward is our way of saying thank you for completing a Health Risk Assessment form with your doctor and committing to making healthy choices.

It’s easy to claim your reward.

1. Call your doctor to schedule your wellness visit.
2. Fill out sections 1 through 3, and your name and address.
3. Take your Health Risk Assessment form with you to your doctor’s appointment. Your doctor will complete section 4 and return it to us.

Gift cards are distributed monthly to members whose doctor has signed and submitted a completed Health Risk Assessment form. Those receiving a discount on their cost-sharing contribution will receive notification on their MI Health account quarterly statement from the state of Michigan.
