As a member of our plan, you’re enrolled in our Disease Management Program if you have:

• Asthma
• Chronic obstructive pulmonary disease
• Diabetes
• Heart disease

As a part of this program, you’ll receive information in the mail. You can request a case manager to assist you in managing your health care needs.
